An exciting opportunity have arisen for a Qualified Social Worker to join the Integrated Community Services (ICS) Team.

The ICS Social Care team collaborates effectively with healthcare professionals in both hospital and community environments. The primary objective of the service is to facilitate the safe discharge of Shropshire residents from hospital by ensuring the provision of appropriate care, as well as offering guidance to promote independent living whenever possible. Adhering to a ‘home first’ and strengths-based approach, the team aims to enhance individuals' autonomy and minimise dependence on paid services where feasible.
