Join our Integrated Community Services team as a Social Care Practitioner, supporting adults to live independently and safely following a stay in hospital. We're looking for someone who meets the specification and who is motivated to supporting vulnerable people to maximise independence using a home first approach.

You'll bring a person-centred approach, strong communication skills, and the ability to work collaboratively with professionals across health and social care. An understanding of the needs of vulnerable adults and relevant legislation is essential.
