Carer Support Practitioner
Join our dedicated team supporting people with learning disabilities and autistic people to live fulfilling and independent lives.
Working across community settings and hospital environments, you will deliver person-centred, strengths-based support that promotes choice, wellbeing and positive outcomes. You will work collaboratively with individuals, families, carers and partner agencies to assess needs, develop and review support plans, facilitate timely hospital discharges, and ensure access to the right support at the right time.
The role requires a commitment to early intervention, prevention and least restrictive practice, alongside maintaining accurate records, contributing to service development, participating in duty and emergency response rotas, and working in line with relevant legislation, policies and professional standards.
